1.    One Ballot per person.

2.    If the place you are voting for has more than one location, please include the street.

3.    All ballots must be at least 100% completed and must include name, telephone number and email address of the voter, for verification purposes only.

4.    Ballots must be received or time stamped by 5 p.m. Friday, June 20th 2008.

5.    Prize awarded through random drawing. Must be 18 year or older to be eligible for prize.

6.    Employees or the Recorder and family member may vote, but are not eligible for prize.
